Junior Protection/Student Protection/Student Sports Injury/GPA (For Student) Claim

Junior Protection Plan, Student Protection Plan, Student Sports Injury Plan and Group Personal Accident (For Student) Plan have the same claim procedure. If your child is covered by any of these plans, here are the steps for submitting a claim.

Before submitting

  • For smoother claims processing, please submit your claim within 30 days from the date of the occurrence.

Death claim

Step 1: Prepare required documents

  1. Certified true copy of death certificate
  2. NRIC or relevant identification documents (e.g. passports, birth certificates)
  3. Last will of the deceased, if any

  3. Newspaper clipping and police report on the death
  4. Last will of the deceased, if any
  1. Certified true copy of death certificate
    • The original death certificate must be certified by your lawyer or any notary public.
  2. Letter from Immigration and Checkpoint Authority (ICA)
    • This letter is issued by ICA for Singaporeans or Permanent Residents who died overseas. It confirms receipt of the Singapore IC, passport and overseas death certificate.
  3. Repatriation report (if body was repatriated to Singapore for cremation/burial)
  4. NRIC or relevant identification documents (e.g. passports, birth certificates)
  5. Proof of claimant's relationship with deceased individual
    Claimant Documents Required
    Spouse Marriage certificate
    Parent Birth certificate of deceased
    Child Birth certificate of claimant
    Sibling Birth certificates of deceased and claimant
  6. Last will of the deceased, if any
  7. Newspaper clipping and police report on the death
All documents submitted must be in English. Any documents in foreign languages must be officially translated to English by a certified translator/interpreter.

After claim submission

We settle most claims within 14 working days after we receive all documents required.

For claims which require further clarification, we will need more time to process your claim. For such cases, we will keep you informed.

Disability claim

  1. Inpatient discharge summary
  2. Original final hospital bills and receipts [1]
  3. Accident reports from school/centre
  4. Police report, if applicable
  5. If you have submitted a claim to any third party who has reimbursed your bills (e.g. other policies / insurers / your employers), please submit copies of the following documents:
    1. Reimbursement letter
    2. Discharge voucher
    3. Payslip reflecting the medical expense deduction (for civil servants)

  1. Original final /medical bills and receipts [1]
  2. Hospital discharge summary (if there is hospitalisation/surgery)
  3. Medical reports, if available
  4. Accident reports from school/centre
  5. Police report, if applicable
  6. If you have submitted a claim to any third party who has reimbursed your bills (e.g. other policies / insurers / your employers), please submit copies of the following documents:
    1. Reimbursement letter
    2. Discharge voucher
    3. Payslip reflecting the medical expense deduction (for civil servants)

Please note that the list of documents above is not exhaustive. Other documents may be requested if necessary.

  1. How to tell if your hospital/medical bills are original and final:
    a. The bill is a final bill, not an estimated or interim bill.
    b. The bill is an original copy, not a duplicate or a photocopy/scanned copy.
    c. There is no outstanding amount due to the medical institution.
    d. The amount covered by Medisave is approved, if applicable.
    e. The amount covered by MediShield / your Private Shield plan is reflected on the bill, if applicable.
