Dependants’ Protection Scheme (DPS) Claim

Here is the process for submitting a Death or Terminal Illness / Total Permanent Disability claim under a Dependants’ Protection Scheme (DPS).

Before submitting

  • For death claims, you must be the insured individual’s nominee or closest next-of-kin.
  • Terminal Illness / Total Permanent Disability claims should be submitted within six months from the date of being certified as terminally ill or being totally and permanently disabled.

Submitting the claim

Step 1: Prepare required documents

  1. Certified true copy of death certificate
  2. NRIC or relevant identification documents (e.g. passports, birth certificates)
  3. Proof of claimant's relationship with deceased individual
    Claimant Documents Required
    Spouse Marriage certificate
    Parent Birth certificate of deceased
    Child Birth certificate of claimant
    Sibling Birth certificates of deceased and claimant
  4. Last will of the deceased, if any
  5. Newspaper clipping and police report (for death due to accidental or violent causes)
  1. Certified true copy of death certificate
    • The original death certificate must be certified by your lawyer or any notary public.
  2. Letter from Immigration and Checkpoint Authority (ICA)
    • This letter is issued by ICA for Singaporeans or Permanent Residents who died overseas. It confirms receipt of the Singapore IC, passport and overseas death certificate.
  3. Repatriation report (if body was repatriated to Singapore for cremation/burial)
  4. NRIC or relevant identification documents (e.g. passports, birth certificates)
  5. Proof of claimant's relationship with deceased individual
    Claimant Documents Required
    Spouse Marriage certificate
    Parent Birth certificate of deceased
    Child Birth certificate of claimant
    Sibling Birth certificates of deceased and claimant
  6. Last will of the deceased, if any
  7. Newspaper clipping and police report (for death due to accidental or violent causes)
All documents submitted must be in English. Any documents in foreign languages must be officially translated to English by a certified translator/interpreter.
  1. Attending Medical Practitioner's Statement, to be completed by the claimant and the attending doctor
  2. Medical reports, investigations reports, laboratory reports and hospital discharge summary
  3. Medically boarded out letter, if any
  4. Newspaper clipping and police/accident report, if applicable
  5. Termination letter from last employer or CPF statement showing last employment contribution
  6. Statement from CPF Board showing CPF contributions for the past 15 months
  7. NRIC or relevant identification documents (e.g. passports, birth certificates)

Step 3: Submit the completed claim form and required documents

You can submit your claim form and necessary documents through any of these channels:

  • At an Income branch
  • To your insurance adviser
  • By post to

After claim submission

We settle most claims within 14 working days after we receive all documents required.

For claims which require further clarification, we will need more time to process your claim. For such cases, we will keep you informed.
