Disability Claim (ElderShield, PrimeShield)

Here are the steps for making a claim under an ElderShield, PrimeShield plan.

Before submitting

  • If you are not able to complete the required forms, your immediate family member or caregiver can complete them on your behalf.
  • You will be required to pay a subsidised fee for the medical assessment.
  • Please ensure the following criteria are met:

Inability to perform three or more Activities of Daily Living (ADLs) as listed below:

Washing The ability to wash in the bath or shower (including getting into and out of the bath or shower) or wash by other means.
Dressing The ability to put on, take off, secure and unfasten all garments and, as appropriate, any braces, artificial limbs or other surgical or medical appliances.
Feeding The ability to feed oneself food after it has been prepared and made available.
Toileting The ability to use the lavatory or manage bowel and bladder function through the use of protective undergarments or surgical appliances if appropriate.
Mobility The ability to move indoors from room to room on level surfaces.
Transferring The ability to move from a bed to an upright chair or wheelchair, and vice versa.

Submitting the claim

  1. Complete claim form
    Please print and complete the claim form.
  2. Make appointment for medical assessment
    Book an appointment with one of the appointed assessors for a medical assessment.

  3. Go for medical assessment
    Please bring along the completed claim form for the appointment.
    After the medical assessment is done, the assessor will then return the assessment report together with the completed claim form to Income for processing.

After claim submission

We settle most claims within 14 working days after we receive all documents required.

For claims which require further clarification, we will need more time to process your claim. For such cases, we will keep you informed.
