Disability Claim - TPD / PPD

Here is a guide to submitting disability claims under individual health or life insurance plans.

Are you a Dependants’ Protection Scheme (DPS) policyholder?


This page is for disability claims under individual non-DPS plans. For information on claiming under DPS, please refer to our DPS claim page.

Before submitting

  • If possible, claims should be submitted within six months from the date of the injury or disease that gave rise to the disability.

Submitting the claim

Step 1: Prepare required documents

  1. Attending Medical Practitioner's Statement, to be completed by the claimant and the attending physician
  2. Medical reports, investigation reports, laboratory reports and hospital discharge summary
  3. Medically boarded out letter, if any
  4. Newspaper clipping and police/accident report, if applicable
  5. NRIC or relevant identification documents (e.g. passports, birth certificates)

Step 2: Complete claim form

Please print and complete the form required for your plan.

Income Family MicroInsurance and Savings Scheme
Total and Permanent Disability claim form (IFMISS)

All other plans
Medical/Accident/Living/Total & Permanent Disability claim form

Step 3: Submit the completed claim form and required documents

You can submit your claim form and necessary documents through any of these channels:

  • At an Income branch
  • To your insurance adviser
  • By post to

After claim submission

We settle most claims within 14 working days after we receive all documents required.

For claims which require further clarification, we will need more time to process your claim. For such cases, we will keep you informed.
